Consent For Nasal Flu Vaccine 2yrs to <18yrs

Our records show that you are eligible for the nasal flu vaccine. It helps protect against flu and reduces the risk of serious illness and complications. Common side effects include a runny or blocked nose, headache, tiredness, or mild fever, which usually settle within a few days. Serious allergic reactions are very rare. Benefits of vaccination in preventing severe flu outweigh this very small risk.

By booking an appointment you are also consenting to receiving a vaccine at the appointment.

Do not automatically book an appointment if you have had any severe allergic reactions to a previous vaccine or any of its ingredients or are feeling unwell. Please speak to a Nurse or Doctor before booking an appointment.
